Someone posted this on Slickdeals. I just filled out the survey and used the store code provided and it appears to have worked! 
Here is the Slickdeals post: “I got breakfast at Chick-fil-a this morning and found a small plastic bag attached that said Free lunch details inside, inside it said to go to www.ceoro-survey12.com and enter in the store number found on the back for a free Chick-fil-a Combo meal.. The reason I am posting this is I was able to use my Store code #BX1-57-17 more than once and from the same PC since my buddy who bought breakfast was pissed he did not get one he tried it and it worked for him also, I can’t find a list of store numbers but they all might have this going on right now. This store is located in Celebration Fl, I am not sure if the coupon I get will only work at this store they said it would take 5 weeks to send out the coupon.”
